The NTSB says the air traffic controller on duty during the crash of Comair flight 5191 had only slept two hours prior to beginning work. The Safety Board earlier revealed the controller had turned away from the accident aircraft, a Bombardier CRJ-200 (or CRJ-100 see our previous story), N431CA, once the plane was cleared to take off.

The Federal Aviation Administration (FAA) also admitted the single controller violated its two controller and distributed duty policy for airport operations like those at Blue Grass Airport in Lexington, Kentucky.
